Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice LPN to join our team serving Central Palm Beach County. Work hours are 8:30 am to 5:00 pm with weekend availability, delivering patient-centered hospice care in home and facility settings.

Under the supervision of the Registered Nurse and primary physician, you will assess patient and family needs, coordinate individualized care plans focused on comfort and dignity, and provide teaching, emotional support, and

Qualifications

• Must hold an active Florida LPN license.
• Current Basic Life Support (BLS) certification.
• Completion of 30-hour IV course per Florida Board of Nursing requirements is a plus.

• Assess patient and family needs under RN and physician guidance.
• Coordinate and provide individualized care plans prioritizing comfort and dignity.
• Provide emotional support and teaching, including overnight care to improve quality of life.
