Job summary

Trustbridge Hospice, part of Empath Health, seeks a caring LPN to provide Continuous Care visits across Broward County. The role supports patients and families during critical moments, delivering compassionate, high-quality home-based care with a 7:00 am to 7:30 pm schedule and alternating weekends.

Qualifications

• Current license to practice as an LPN in Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting Florida Board of Nursing requirements is a plus.
• Adequate and reliable transportation with adequate insurance and a valid Florida driver’s license.

Responsibilities

• Assess patient and family needs and help develop personalized care plans.
• Provide monitoring, intervention, and teaching to enhance quality of life.
• Deliver overnight care and provide emotional support to patients and families.
• Coordinate with the Registered Nurse and primary physician to implement care.
• Ensure compassionate, personalized service and a positive experience for every patient and family.
