Job summary

Home Health & Hospice Care in Merrimack, NH is seeking a Licensed Nursing Assistant for Community Hospice House 2nd shift (3:00pm - 11:00pm) with weekend and holiday rotation.

You will provide personal care, assist with mobility, observe patient status, document care activities, and collaborate with an RN and the care team to support comfort and safety in a compassionate, patient-centered environment.

Qualifications

• Licensed Nursing Assistant in good standing with the NH Board.
• CPR certification.
• Background check and motor vehicle record required.
• High School diploma or higher.
• Reliable, compassionate, and committed to patient-centered care.

Responsibilities

• Provide personal care such as bathing, dressing, grooming, toileting, and transfers.
• Assist with mobility using proper body mechanics.
• Observe and report changes in patient condition and status.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe environment following safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
• Uphold professional boundaries and provide excellent customer service.
