Job summary

Home Health & Hospice Care in Merrimack, NH is seeking a Licensed Nursing Assistant for Community Hospice House on the 1st shift (7:00am - 3:00pm), 32 hours per week with every other weekend and holiday rotation. The role supports patient comfort under RN supervision and collaborates with the care team.

Qualifications include NH LNA license, CPR certification, and a clean background check. High school diploma or better required; drivers license also needed. Equal opportunity employer.

Qualifications

• Must be a Licensed Nursing Assistant in good standing with NH Board of Nursing.
• CPR certification required and successful background check.
• Reliable, compassionate, and committed to patient-centered care.

Responsibilities

• Provide personal care services including bathing, dressing, grooming, toileting, and safe transfers.
• Use proper body mechanics and devices to assist mobility.
• Observe and report changes in patient status and intake/output.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe environment following safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
