Job summary

The Licensed Nursing Assistant role at Home Health & Hospice Care's Community Hospice House is a 2nd shift position in Merrimack, NH. You will deliver compassionate personal care under an RN's supervision, supporting patient comfort, safety, and well-being.

Responsibilities include bathing, grooming, safe transfers, documenting observations, and maintaining a clean, respectful environment. A NH-licensed CNA with CPR certification and reliable teamwork is preferred; weekend/holiday rotation

Qualifications

• Provide personal care services including bathing, dressing, grooming, toileting, and transfers.
• Observe and report changes in physical or mental condition and care needs.
• Maintain a clean, safe environment following aseptic and safety standards.
• Document care using the agency's system and communicate with the care team.

Responsibilities

• Support patient comfort and safety under the direction of an RN.
• Assist with mobility and transfers using proper body mechanics.
• Provide companionship and respond to patient needs.
• Follow care plans and report concerns promptly.
