Job summary

Home Health & Hospice Care is seeking a Licensed Nursing Assistant for the Community Hospice House on the Merrimack campus in New Hampshire. The 3rd shift (11:00pm–7:00am) offers 32 hours per week with weekend and holiday rotation per policy.

You'll work under an RN to support patient comfort and safety in a compassionate, team‑oriented setting. Ideal candidates hold an NH LNA license, CPR certification, and a valid driver's license; prior home care or dementia care experience is a plus.

Qualifications

• Must demonstrate patient-centered care and safety.
• Follow care plans and document changes accurately.
• Collaboration with RNs and care team required.

Responsibilities

• Provide personal care including bathing, dressing, grooming, toileting, and transfers.
• Observe and report changes in patient condition and intake/output.
• Support patient comfort, safety, and well-being under RN direction.
• Maintain a clean, safe environment following safety standards.
