Job summary

Home Health & Hospice Care is seeking a Licensed Nursing Assistant for the Community Hospice House on the Merrimack campus in New Hampshire. The 3rd shift (11:00pm–7:00am) offers 32 hours per week with weekend and holiday rotation per policy.

You'll work under an RN to support patient comfort and safety in a compassionate, team‑oriented setting. Ideal candidates hold an NH LNA license, CPR certification, and a valid driver's license; prior home care or dementia care experience is a plus.

Qualifications

• Must demonstrate patient-centered care and safety.

Responsibilities

• Provide personal care including bathing, dressing, grooming, toileting, and transfers.

About the Job:
The Community Hospice House recently completed a transformative expansion and renovation project to meet the growing need for compassionate, end‑of‑life care in southern New Hampshire. The facility added 6 new private patient suites, increasing its total room count from 10 to 16. As a Licensed Nursing Assistant in the Community Hospice House, you’ll play a key role in delivering compassionate, high‑quality personal car. You’ll work under the direction of an RN and collaborate closely with the care team to support patient comfort, safety, and well‑being. This role is hands‑on, relationship‑driven, and essential to helping patients remain safe and comfortable.

What You’ll Do:

• Provide personal care services including bathing, dressing, grooming, toileting, and safe transfers.
• Use mechanical devices and proper body mechanics to ensure safe mobility assistance.
• Observe and report changes in physical or mental condition, intake/output, and overall patient status.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe, and healthy environment following aseptic and safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
• Uphold professional boundaries and represent the agency with excellent customer service.

Who You Are:

• A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
• CPR certified, with a satisfactory background check and motor vehicle record.
• Reliable, compassionate, and committed to delivering exceptional patient‑centered care.
• Preferred: experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post‑surgical needs; ability to perform PT/INR checks.
