Job summary

Mercy Health Home Care and Hospice by Compassus is seeking an Inpatient Unit (IPU) Registered Nurse Case Manager to lead the care coordination and education for patients and families. You will work with the Interdisciplinary Team to ensure comfort, manage symptoms, and coordinate services in a flexible schedule including after-hours on-call support.

The role requires an RN license in the relevant state, CPR certification, and a Nursing degree.

Qualifications

• Active and unencumbered RN license in state(s) of employment.
• CPR certification required.
• Bachelor’s or Associate degree in Nursing.
• Hospice, oncology, home health, or long-term care experience preferred.
• One year of nursing practice or equivalent experience preferred.

Responsibilities

• Coordinate clinical, psycho-social and spiritual services as indicated by Plan of Care.
• Collaborate with IDT to meet patient needs with interventions.
• Explain hospice services and Medicare benefits to patients and families; obtain consent and election of benefits.
• Submit documentation to facilities after patient visits.
• Supervise Hospice Aides and LPNs/LVNs and adjust assignments as needed.
