Job summary

Compassus is seeking an IPU Registered Nurse Case Manager in Alabama to coordinate clinical, psycho-social, and spiritual services through the case management model. You will collaborate with the IDT to assess needs, educate patients and families, and ensure continuity of care per the Plan of Care.

The role includes after-hours on-call responsibilities, bedside documentation, and supervising Hospice Aides/LPNs to promote optimal patient outcomes in a home-like setting.

Qualifications

• Associates or Bachelor's degree in Nursing required.
• Active and unencumbered RN license in state(s) of employment.
• Current CPR certification required.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Coordinates clinical, psycho-social and spiritual services as indicated by Plan of Care through case management model.
• Collaborates with IDT to meet patient needs and coordinate hospice care.
• Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents.
• Maintains updated, comprehensive care plans reflecting current problems, goals and interventions.
• Performs supervisory duties for Hospice Aides and LPN/LVN staff and adjusts assignments to promote outcomes.
• Records observations, interventions, and pertinent information; ensures documentation at bedside.
