Job summary

Compassus is seeking an Inpatient Unit Registered Nurse Case Manager to coordinate clinical, psycho-social, and spiritual services for patients under hospice care within the IPU. You will work with the IDT to ensure comprehensive care plans, educate families, and manage documentation while supporting after-hours on-call coverage.

Qualifications

• Associate or Bachelor’s degree in Nursing required.
• Minimum of one year nursing practice or equivalent experience preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Coordinate clinical, psycho-social and spiritual services per Plan of Care through the case management model.
• Collaborate with IDT to meet patient needs for clinical, psycho-social and spiritual interventions.
• Explain hospice services and Medicare benefits to patients and families; obtain Informed Consent and Election of Benefits documents.
• Submits appropriate documentation and paperwork to facilities at the completion of patient visits.
