Compassus is seeking a Hospice Registered Nurse / RN Case Manager to support patients and families in home-based hospice care. You will coordinate care, build trusting relationships, and empower caregivers with essential education to enhance comfort, dignity, and quality of life.

Qualifications

• Active RN license in the state you work.
• Two years of nursing experience; hospice experience preferred.
• Current CPR/BLS certification and valid driver’s license.

Responsibilities

• Coordinate patient care and case management in hospice settings.
• Educate caregivers on at-home care and comfort measures.
• Collaborate with the care team to plan and monitor patient treatment.
• Document care plans and patient outcomes clearly and timely.
