Job summary

AccentCare is seeking a dedicated RN IPC in Hospice to join our Wilmington Hospital IPC team. You will provide comprehensive nursing care, coordinate with the interdisciplinary team, and educate patients and caregivers on medications and end-of-life planning.

This on-site role requires compassion, reliability, and the ability to handle on-call duties. Schedule is 3/12s Nights 7:00pm - 7:30am; prior hospice or home health experience is preferred, with CPR certification and a valid state RN

Qualifications

Must be a licensed Registered Nurse in the state of practice; CPR certification required.
Experience in hospice, home health, medical-surgical, or geriatrics desired; 1 year preferred in CT.
Willingness to serve on-call and uphold the organization’s values and mission.

Responsibilities

Provide comprehensive nursing care with individualized care plans.
Coordinate with physicians and the interdisciplinary team to ensure compassionate care.
Maintain accurate documentation and update patient records during shifts.
Educate patients and caregivers on medications, mobility, and end-of-life prep.
Offer emotional and physical support at the time of death and participate in 24/7 coverage.
Attend team meetings, supervise aides, and adhere to nursing regulations.
