Qualifications

Responsibilities

• Provide comprehensive nursing care by assessing, planning, implementing, and evaluating care plans for hospice patients and families.
• Coordinate with physicians and the interdisciplinary team to ensure seamless, compassionate care.
• Maintain accurate documentation and update patient records during shifts.
