Job summary

AccentCare, Inc. is hiring a Registered Nurse IPC in Newark, DE to provide nursing care for hospice patients and their families during evening/night shifts.

You will coordinate with physicians and the care team, document care plans and updates, educate patients and caregivers, and participate in 24/7 coverage with the hospice staff. Qualified candidates are licensed RNs, with hospice/home health experience; willing to be on-call, have a valid driver’s license, and meet physical demands.

Qualifications

• Must be a licensed RN in the state of practice; associate degree preferred; CPR certification required.
• Requires hospice/home health/medical-surgical experience; one year in CT.
• Willingness to serve on-call and uphold the organization’s values and mission.
• Must have a valid driver’s license, reliable vehicle, and meet state auto insurance requirements.
• Physical demands include frequent lifting and variable working conditions.

Responsibilities

• Provide comprehensive nursing care by assessing, planning, implementing, and evaluating hospice patient care plans.
• Coordinate with physicians and the interdisciplinary team to ensure compassionate care and proper resource use.
• Maintain accurate documentation and update patient records during shifts.
• Educate patients and caregivers on medication, personal care, mobility, and end-of-life preparation.
• Offer emotional and physical support at time of death and participate in 24/7 coverage with the hospice team.
• Attend team meetings and assist with supervision, training and regulatory compliance.
