Job summary

AccentCare in Newark, DE is seeking an experienced RN IPC Hospice to join our inpatient unit team. This full-time role offers 7pm–7:30am shifts (3/12s) with every other weekend rotation, requiring an active RN license and hospice/home health experience.

You will assess, plan, and coordinate compassionate end-of-life care, document thoroughly, support patients and families, and collaborate with physicians and the interdisciplinary team.

Qualifications

• Licensed RN in the state of practice.
• Hospice, home health, or medical-surgical experience preferred.
• Willingness to be on-call and support the mission of compassionate care.

Responsibilities

• Assess, plan, implement, and evaluate hospice care plans.
• Coordinate with physicians and the care team to ensure compassionate care.
• Maintain timely documentation and updated patient records.
• Educate patients and families on medications, care, and end-of-life preparation.
• Support patients and caregivers during times of death and provide 24/7 coverage as needed.
