Job summary

AccentCare, Inc. is seeking an RN IPC Hospice to join the inpatient hospice unit in Berwyn, IL. This PRN position offers twelve-hour shifts, days or nights, and local coverage.

You will provide comprehensive nursing care, coordinate with the team, maintain patient records, and support families through end-of-life care. A valid RN license and hospice/home health experience are preferred; CPR required.

Qualifications

• Must be a licensed RN in the state of practice.
• CPR certification is required.
• Experience in hospice, home health, medical-surgical, or population-specific care is preferred.

Responsibilities

• Provide comprehensive nursing care and individualized care plans.
• Coordinate with physicians and the interdisciplinary team for seamless care.
• Maintain accurate patient documentation throughout the shift.
• Educate patients and caregivers on medications, mobility, and end-of-life planning.
• Support hospice team with on-call coverage and meetings.
