Job summary

Beth Israel Lahey Health in Massachusetts is seeking a Registered Nurse to support hospice services on weekend day shifts. You will manage referrals, coordinate with a multidisciplinary team and document patient information in Epic.

The role requires MA RN licensure, 2–3 years hospice experience, strong communication, and the ability to work independently with minimal supervision. You must meet safety and HIPAA guidelines and participate in weekend rotation.

Qualifications

  • Must be a licensed nurse in Massachusetts.
  • Bachelor’s degree preferred.
  • 2–3 years’ hospice experience required.
  • Strong observation, problem solving and critical thinking skills.
  • Excellent verbal and written communication; proficient computer skills with EMR.

Responsibilities

  • Responds to requests promptly and positively.
  • Triages referrals based on hospice needs and assigns visits.
  • Inputs initial intake information into Epic for admission.
  • Maintains confidentiality per HIPAA guidelines.
  • Works with weekend Administrator On Call to address patient issues.

Weekend, Day Shift

Duties/Responsibilities
  • Responds to all requests in a positive manner.
  • Working knowledge of the skills and service provided by each member of the multidisciplinary team and the ability to relate these to identified patient needs.
  • Prioritize and assigns referrals based on patient’s hospice needs.
  • Understands and integrates relevant hospice standards, regulations and practices to make decisions regarding appropriateness for care.
  • Accepts and documents verbal orders related to the initiation of services. Inputs initial information into Epic to begin the admission process.
  • Elicits and gathers accurate and pertinent information regarding the patient’s condition at the time of intake, to accurately identify critical information needed to initiate hospice care.
  • Forward all referral information to the appropriate team for timely initiation of services.
  • Responds to telephone requests for information and interprets services to callers.
  • Triages patient calls, advises interventions based on clinical knowledge and/or makes skilled nurse visit to assess patient.
  • Acts as a resource to patients, families, and caregivers as needed.
  • Works collaboratively with the weekend Administrator On Call (AOC) to address patient related issues that arise. Triages unanticipated patient needs and assigns visits accordingly, schedules patient visits and makes nurse visits as needed.
Communication
  • Communicates with the physician regarding the patient’s needs and obtains/receives physician's orders as required.
  • Provides timely communication to the clinical team whenever there are unusual requests or complex situations/orders.
  • Practices confidentiality principles set by the agency and federal HIPAA guidelines.
Documentation
  • Must have attention to detail when taking, processing and documenting referral requests throughout the intake process.
  • Proficiency with common office software programs and ability to learn other software products including, but not limited to, Epic.
  • Completes documentation in accordance with agency timeliness policies.
Professional Development
  • Maintains and updates clinical knowledge and skills based on current nursing practice.
  • Adheres to departmental requirements for required training.
  • Identifies specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them.
Additional Duties
  • Participates in the weekend/holiday rotation as defined by BIDP.
  • Participates in in-service programs and supervises, teaches and precepts hospice nursing staff.
Education/Experience Required
  • Professional Nurse from a Graduate of National League for Nursing accredited school of nursing.
  • Bachelor’s degree preferred.
  • 2-3 years’ experience in hospice services.
  • Comprehensive knowledge of hospice regulations.
  • Demonstrates excellent observation and problem solving/critical thinking skills.
  • Well‑developed verbal and written communication skills; essential computer skills to work with EMR.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Self‑directed and able to work with minimal supervision.
  • Shows strong ability to organize and prioritize workload independently.
Registration/Certification
  • Registered Nurse licensure in the state of Massachusetts.
Physical/Environmental Requirements

This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.

Pay Range

$37.00 - $80.00

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
