Job summary

Beth Israel Lahey Health seeks a Hospital Based Admissions Nurse to plan and deliver hospice care using the nursing process, coordinating with hospital providers and the Hospice IDT. You will assess, teach, counsel patients and families, and ensure appropriate GIP referrals and admissions.

The role requires MA RN licensure, BSN preferred, BLS, and a minimum of 3 years hospice experience with strong communication and EMR skills.

Qualifications

• Minimum 3 years Hospice Experience.
• Inpatient hospice experience preferred.
• Hospice or end-of-life care experience required.

Responsibilities

• Plan and deliver care using the nursing process and coordinate with the interdisciplinary team.
• Explain hospice services to patients and families and address questions about care and finances.
• Coordinate preadmission activities and admission processes for GIP level of care.
• Provide symptom management and collaborate with the Hospice Medical Director.
• Conduct daily nursing visits and monitor ongoing eligibility for GIP level of care.
