Beth Israel Lahey Health's Mount Auburn Hospital seeks a Hospital Based Admissions Nurse to determine GIP eligibility, coordinate preadmission activities, and support hospice referral workflows.

You will educate patients and families about hospice services, discuss insurance implications, and collaborate with the care team to ensure smooth transitions and compassionate end-of-life care.

This role requires RN licensure in Massachusetts, hospice experience, and BSN preferred; no weekends.

Qualifications

• Registered Nurse licensed in Massachusetts.
• BSN strongly preferred.

Responsibilities

• Determine appropriateness of GIP level of care and coordinate preadmission activities.
• Increase accessibility to hospice care and manage admission process via effective communication.
• Explain hospice services to patient/family and address questions about status and program.
• Gather information necessary for admission.
• Discuss insurance coverage and costs with patient/family.
• Promote hospice services to ensure GIP referrals to hospice program.
• Coordinate care among hospital team and hospice team.
• Provide symptom management in coordination with Hospice Medical Director.
• Document services and plan continuity of care with nursing team, patient, and family.
• Teach nursing care and health practices to patient and family.
