JourneyCare Hospice is seeking an Intake Coordinator to join our growing care team in Crystal Lake, IL. The role emphasizes timely documentation, clear communication with patients and families, and coordination of admissions for hospice services.

The position offers a supportive culture, comprehensive benefits, and opportunities for growth within a compassionate homecare setting. Monday–Friday hours with occasional weekend coverage.

Qualifications

• Must be 18 years of age or older.
• At least 1 year experience in homecare, hospice, or healthcare-related field preferred.
• Ability to complete necessary documentation accurately, legibly, and timely.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Receive referrals by phone or in person for the designated department.
• Explain benefits to patients, families, and community contacts.
• Assist with finding alternative services when the patient is not a hospice candidate.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure proper forms are available to admissions staff.
• Assist with marketing activities to promote relations with referral sources.
• Assist with completing admission paperwork as needed.
• Adhere to schedule and maintain punctuality and attendance.
• Provide telephone and clerical support to staff as time allows.
• Maintain accurate clinical records and track client goals and progress.

Qualifications

• Be eighteen (18) years of age or older
• At least one (1) year experience in homecare, hospice, and/or healthcare-related field preferred
• Ability to complete necessary documentation appropriately, legibly, and timely
• Ability to handle stressful and delicate situations
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
• Valid driver's license and proof of insurance are required

Hours

Monday-Friday 9:00am-5:30pm - Plus Working Every 3rd Weekend (Saturday and Sunday)

Crystal Lake, IL.

What You'll Do

Pay Range

$20-$28 hourly - offer based on experience.
