Job summary

AccentCare, Inc. in Rosemont, IL is seeking a full-time Admissions Coordinator to manage referrals, enter data into the EMR, and coordinate patient visits to initiate hospice services.

The role requires collaboration with referral sources, physicians, and Seasons staff to ensure timely admissions and high-quality patient care. The ideal candidate will have an associate degree (preferred), 1+ year hospice experience, and strong communication and organizational skills to support a compassionate

Responsibilities

• As Admissions Coordinator, receive referrals and obtain critical information to ensure timely data entry in the EMR.
• Input referrals and basic patient information into the EMR to assess how hospice services may help.
• Coordinate and schedule visits with appropriate staff based on patient condition and family needs.
• Communicate with referral sources, physicians, Seasons staff, and other agencies to initiate hospice services.
• Maintain regular communication during sign-on, consent and admission assessment phases to ensure seamless coordination.
• Maintain accurate EMR documentation and referral statistics.

Responsibilities

As an Admissons Coordinator, you will:

• Receive professional referrals and obtain the critical referral information in the form easiest for the referral source(electronic, fax notification, phone, Hospice Care Consultant (HCC) notification, etc.) to ensure timely responseand data entry in the electronic medical record (EMR) system.
• Receive potential patient and family inquiries regarding Hospice services. Obtain and input referrals into EMR, thebasic information regarding the patient status in order to describe how Seasons may assist, with a goal ofscheduling a visit to initiate the relationship
• Coordinate and schedule visits to ensure the appropriate staff makes initial contact, based on patient conditionand family situation, to meet the professional referral source
• Communicate with referral sources, attending physicians, Seasons staff members, and other agencies asneeded, to coordinate initiation of hospice services
• Maintain regular communication with all involved in sign-on, consent and admission assessment phases toensure seamless coordination of information and services
• Maintain accurate EMR documentation and referral statistics
