Job summary

Hospice of the Chesapeake Inc. seeks a compassionate PRN RN to join the Inpatient Care Center in Pasadena, MD.

You will lead nursing care, case management, and collaborate with social workers, chaplains, CNAs and hospice physicians to provide physical, psychosocial, and spiritual care to patients with life-limiting illnesses. This night-shift role requires MD RN license, 2+ years in med/surg or home care, hospice or critical care preferred, and EMR documentation proficiency.

Qualifications

• Maryland RN license or accepted reciprocal license.
• 2+ years of nursing experience in med/surg or home care.
• Hospice, end-of-life, or critical care experience preferred.
• CHPN certification preferred.
• Familiarity with documenting in an EMR.

Responsibilities

• Provide direct patient care per hospice plan of care.
• Identify and communicate patient problems, priorities, and goals.
• Collaborate with interdisciplinary team to plan care.
• Establish care priorities based on patient condition.
• Document patient teaching and discharge planning.
• Complete all electronic documentation at the point of care; assist field teams when needed.
