CHESAPEAKE HEALTH PARTNERS INC in Pasadena, MD is seeking a compassionate PRN/RN to join our Inpatient Care Center hospice team. You will provide direct nursing care and collaborate with social workers, chaplains, CNAs, and hospice physicians to support patients and families.

This role requires Maryland RN licensure (or reciprocal out-of-state) with 2+ years in med/surg or home care; hospice experience preferred.

Qualifications

• Current Maryland RN license or accepted reciprocal out-of-state nursing licensure; and/or CHPN certification preferred.
• 2+ years of nursing experience in med/surg or home care required, with hospice/end-of-life/critical care experience preferred.
• Hospice certification (CHP) preferred.
• Strong, problem-solving skills with ability to prioritize and delegate.
• Familiarity with documenting in an Electronic Medical Record (EMR).
• Valid driver’s license with acceptable driving record and reliable transportation.
• Ability to meet the physical demands of the position with or without accommodation.
• Influenza vaccination requirement and TB screening per company policy.

Responsibilities

• Provide direct patient care per hospice plan of care, ensuring treatments, medications, and diet as tolerated.
• Identify, communicate and collaborate on patient problems and priorities with the interdisciplinary team.
• Collaborate with patient/family when developing plan of care and set mutual expectations.
• Establish care priorities based on patient condition and needs.
• Ensure patient/caregiver teaching and understanding; incorporate teaching into routine care and discharge planning.
• Complete all electronic documentation at point of care or in patient care area following protocol.
• Assist Field Teams when needed.

Want to join our Team? We are currently looking for a compassionate and dedicated PRN/RN to join our Inpatient Care Center (ICC) team. As an RN in a GIP-setting, you will take the lead in providing nursing care and case management for those with life-limiting illnesses, helping patients and their caregivers navigate end-of-life care with compassion. You will work with your team’s Social Workers, Chaplains, and CNAs, as well as our Hospice Physicians, to provide the physical, psychosocial, and spiritual aspects of care.Office Location: Pasadena, MarylandCoverage Area: Rebecca Fortney Inpatient Care CenterStatus: PRNSchedule: Nights – 7pm – 7:30am - PRN's are required to work a minimum of two (2) shifts per month and one (1) holiday per year.Main Responsibilities:Provide direct patient care sufficient to meet the patient’s total nursing need in accordance with the patient’s hospice plan of care assuring each patient receives treatments, medications, and diet as tolerated.Correctly identifies, communicates and collaborates on patient problems/priorities/issues based upon assessment data, establishes measurable patient goals and communicates initial/ongoing plan of care appropriately to interdisciplinary team. Recognizes changes in clinical status and reports them to the appropriate individual.Collaborates with patient/family when developing plan of care, adjusting communication style as necessary. Establishes priority for patient care based on immediate patient condition and mutually set patient needs/expectations.Ensures patient/caregiver teaching and patient/caregiver level of understanding and response to teaching are documented. Incorporates teaching into routine care, involving patient/caregiver.
