Professional Healthcare Resources seeks a dynamic Hospice RN to join a passionate team of Clinical Professionals in Baltimore. Full-time, part-time, and PRN roles are available across Baltimore, Kensington, Lanham, DC and Virginia branches.

The RN will cover case management, assessments, care planning and evaluation of patient needs for hospice and home health services. The ideal candidate holds a compact RN license or RN license for the operating state(s) and has at least 1 year of nursing

Qualifikationen

• Must have a Compact RN License and/or Registered Nurse licensed in the state(s) of operation.
• Minimum of one year of licensed professional nursing experience preferred; home health or community health experience preferred.
• Experience performing assessments, care planning and case management for hospice/home health patients.

Aufgaben

• Performs initial and ongoing assessments of the client’s needs for home health care or hospice.
• Acts as case manager, coordinating patient/family services with the interdisciplinary team.
• Initiates appropriate nursing procedures and provides referrals as needed.
• Prepares complete clinical documentation per agency policy.
• Communicates changes in patient condition and obtains new orders as required.
• Maintains coordination of services to meet patient goals and ensure cost-effective care.
