Job summary

Hospice of the Chesapeake Inc. Qualifications

• Current Maryland Board of Nursing RN license and thorough knowledge of nursing principles.
• Two or more years of med/surg or home health experience required; hospice experience preferred.
• Proficiency with basic computer software and EHR systems.

Responsibilities

• Admission assessment and care planning with patients and families.
• Conduct skilled nursing visits in various settings and document care.
• Perform death visits and coordinate bereavement information.
• Collaborate with interdisciplinary team and notify physicians of changes.
