Job summary

Four Seasons in Henderson County, NC seeks a Hospital Liaison RN to coordinate between contracted hospitals and the hospice team. This exempt, 8a–4:30p role includes inpatient visits, education to providers, and building partnerships with referral sources.

You will assess, plan, and implement nursing care, guide admissions, obtain consents, and collaborate to ensure quality of life for patients and families.

Qualifications

• Valid North Carolina RN license, unrestricted.
• Minimum 2 years clinical experience preferred.
• Minimum 1 year hospice experience preferred.

Responsibilities

• Serve as Four Seasons ambassador within the community.
• Assess patient’s physical and mental health and report nursing assessments.
• Coordinate discharge planning and collaboration with the care team.
• Develop and implement patient-centered care plans with medical staff.
