Job summary

Four Seasons is seeking a Hospice Registered Nurse to provide nursing coverage for patients and families via phone and in-person visits, including after-hours call support. The role involves coordinating hospice care, assessing patients, and collaborating with case managers to ensure quality, patient-centered care.

Responsibilities include nursing assessments, documentation, teaching, and supervising staff while maintaining compliance with regulations.

Qualifications

• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• HIPAA privacy rules and regulations.
• Experience with electronic medical records systems.
• Excellent written and oral communication skills.
• Ability to work under pressure and meet deadlines.
• Ability to plan, prioritize and coordinate multiple projects.
• Ability to work with diverse departments and teams.
• Flexible hours and team-oriented approach.

Responsibilities

• Provide nursing care to patients, families, and partners via phone and in-person visits.
• Serve as first responder for after-hours symptom-based calls.
• Coordinate patient hospice care with case management as needed.
• Assist with emergency admissions and patient assessments.
• Record and report nursing assessments and care plans.
• Supervise and train other staff in nursing procedures.
