Four Seasons is seeking an Admissions RN to serve as an ambassador for our hospice mission, handling admissions and occasional patient visits with a focus on assessing needs and coordinating care. Qualifications

• Must be willing and able to care for patients with infectious disease processes when accommodations aren’t required
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
• Knowledge of HIPAA privacy rules and regulations
• Knowledge of electronic medical records systems
• Demonstrate excellent written and oral communication skills
• Ability to work under pressure to meet deadlines
• Ability to take initiative and utilize innovative techniques and ingenuity
• Ability to interact with various departments throughout the organization
• Ability to work flexible hours, if needed
• Ability to participate as a team player to coordinate projects
• Ability to plan, prioritize and coordinate multiple projects
• Demonstrate skill/expertise in pain and symptom management

• Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation

Responsibilities

• Perform 2–4 admissions daily depending on shift length and patient complexity
• May perform patient visits providing direct nursing care as needed
• Complete comprehensive nursing assessments
• Educate patients and families about care plans and services
• Build and sustain partnerships with referral sources in the community
• Coordinate with other health care providers to develop patient-centered plans
• Provide teaching and counseling about health and treatment regimens
• Record and report nursing assessments and patient responses
• Supervise and mentor staff delivering nursing services
• Ensure safe and effective nursing care and maintain documentation
