Delaware Hospice is seeking a full-time Hospital Liaison based in Newark, DE to cover hospitals in New Castle County. The role serves as a clinical and business development partner, promoting access to palliative, dementia, Katybug pediatric, and hospice care services across assigned healthcare systems.

Qualifications

• Current professional nursing license in the state or jurisdiction.
• Graduate of an approved school of nursing.
• Bachelor’s degree in nursing preferred.
• Certification in hospice nursing preferred.
• Minimum of two years of nursing experience, preferably in hospice, home care, hospital, or another healthcare setting.
• Strong communication, relationship-building, collaboration, organization, and presentation skills.
• Ability to work effectively with physicians, healthcare-system leaders, interdisciplinary teams, patients, and families.
• Ability to understand healthcare-system priorities and translate clinical expertise into collaborative, patient-centered solutions.

Responsabilités

• Serve as a clinical and business development partner within assigned healthcare systems, service areas, and growth markets.
• Collaborate with hospital leaders, physicians, case management, social work, nursing, palliative care, and other key stakeholders to improve access to Delaware Hospice’s services.
• Strengthen long-term partnerships between Delaware Hospice and healthcare systems across assigned geographies.
