Job summary

Delaware Hospice seeks a full-time Hospital Liaison to cover a Sussex County hospital. The role blends clinical care with business development, strengthening access to hospice and related services through trusted hospital partnerships.

The ideal candidate has an active nursing license, at least two years in healthcare, and strong communication and collaboration skills to work with physicians, case managers, and leaders across care teams.

Qualifications

• Current professional nursing license in the state or jurisdiction required for the assigned service area.
• Graduate of an approved school of nursing.
• Bachelor's degree in nursing preferred.
• Certification in hospice nursing preferred.
• Minimum of two years of nursing experience, preferably in hospice, home care, hospital, or another healthcare setting.

Responsibilities

• Serve as clinical and business development partner within assigned healthcare systems, service areas, and growth markets.
• Support timely patient assessment and admission processes.
• Collaborate with hospital leaders, physicians, case management, social work, nursing, palliative care, and other stakeholders to improve access to Delaware Hospice’s services.
• Identify opportunities to strengthen partnerships and access to Delaware Hospice’s full suite of services.
