Job summary

AccentCare is seeking a Clinical Liaison RN in Newark, Delaware, full-time to support hospice admissions and coordinate care transitions for patients and families. The role combines nursing expertise with business-savvy interactions with physicians and healthcare leadership to ensure timely, compassionate hospice services.

The candidate will identify patient needs, facilitate transfers, and partner with healthcare professionals to optimize patient outcomes in a hospital-to-hospice transition.

Qualifications

• One (1) year of RN experience in local market with knowledge of governmental home health regulations is preferred.
• Licensed RN currently licensed in the state of operation.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with Hospice Care Consultants to develop and implement individualized marketing plan.
• Maintain visibility and relationships with inpatient facilities and physician groups.
• Act as liaison with Medical Directors, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department and Team Directors.
• Provide consultation on hospice and palliative care.
