Job summary

Delaware Hospice seeks a full-time Hospital Liaison to cover Kent and Sussex Counties, combining clinical expertise with business development to improve access to hospice services. You will collaborate with hospital leaders, physicians, and care teams to support transitions and strengthen partnerships across the region.

The role emphasizes patient assessment, admission support, and growth opportunities in palliative, dementia, Katybug pediatric, and hospice care services.

Qualifications

• Current nursing license in the state or jurisdiction for the assigned service area, with ability to expand licensure as needed.
• Graduate of an approved school of nursing.
• Bachelor's degree in nursing preferred.
• Certification in hospice nursing preferred.
• Minimum of two years of nursing experience, preferably in hospice, home care, hospital, or another healthcare setting.
• Strong communication, relationship-building, collaboration, organization, and presentation skills.

Responsibilities

• Serve as a clinical and business development partner within assigned healthcare systems, service areas, and growth markets.
• Represent Delaware Hospice to hospital leaders, physicians, case management, social work, nursing, palliative care, and other stakeholders.
• Improve access to Delaware Hospice services and strengthen long-term partnerships across geographies.
