Delaware Hospice is seeking a full-time Hospital Liaison to cover New Castle County hospitals. The role blends clinical expertise with business development, strengthening access to palliative, dementia, Katybug pediatric support, and hospice services across assigned systems.
You will work with hospital leaders and care teams to improve transitions and long-term partnerships. The ideal candidate has a nursing degree, an active RN license, and 2+ years in hospice or related healthcare settings,
Qualifikationen
- Current professional nursing license in the state or jurisdiction.
- Graduate of an approved school of nursing.
- Bachelor's degree in nursing preferred.
- Certification in hospice nursing preferred.
- Minimum of two years of nursing experience, preferably in hospice, home care, hospital, or another healthcare setting.
- Strong communication, relationship-building, collaboration, organization, and presentation skills.
- Ability to work effectively with physicians, healthcare-system leaders, interdisciplinary teams, patients, and families.
Aufgaben
- Serve as a clinical and business development partner within assigned hospital systems and markets.
- Improve access to Delaware Hospice services, including palliative and hospice care.
- Facilitate care transitions and coordinate with hospital stakeholders to strengthen partnerships.
- Represent Delaware Hospice to hospital leaders, physicians, case management, social work, and nursing teams.
