Job summary

AccentCare Inc. in Newark, Delaware seeks a full-time Clinical Liaison RN to support hospice admissions and transitions with hospitals, physicians and care teams. The role emphasizes compassionate patient advocacy and strategic outreach to referral sources.

The Clinical Liaison will identify patient needs, coordinate care plans, and ensure timely admission to AccentCare Hospice services, while maintaining compliance with home health regulations and delivering superior patient and family support.

Qualifications

• RN with 1 year in local market and knowledge of home health regulations.
• Experience in care plan development techniques.
• Licensed RN in the state of operation.
• Must have a driver’s license and insured vehicle.

Responsibilities

• Provide community and provider education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Develop and implement individualized hospice marketing plans.
• Maintain visibility and relationships with inpatient facilities and physician groups.
• Act as liaison with Medical Directors and Hospice Physicians for patients and families.
• Make inpatient visits at contracted facilities and coordinate with Admissions and Team Directors.
• Provide consultation on hospice and palliative care.
