Job summary

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking an RN – New Market in Eau Claire, WI. This part-time 0.8 FTE role includes a split shift, Monday–Friday, 10:00a–7:00p, with on-call, weekend, and holiday rotations, focused on patient-centered care in new markets.

You will assess patients and families, develop and revise plans of care, coordinate with the interdisciplinary team, and support admission to homecare services, while upholding Agrace philosophy

Qualifications

• Current, unrestricted RN license in the state of employment.
• One year nursing experience preferred.
• Ability to develop and apply knowledge of hospice and end-of-life processes.

Responsibilities

• Perform patient and family assessments for hospice and palliative care.
• Develop and revise the plan of care with the IDT.
• Coordinate admissions to homecare in new markets.
• Provide nursing interventions and pain management.
• Document care and communicate with the care team.
