Agrace in Eau Claire, WI is seeking a Part-Time RN – New Market to support hospice and homecare services in new markets. The role focuses on patient and family-centered care, admissions, and coordination with the interprofessional team.

You will assess, plan, implement, and document care, supervise CNA/LPN staff, and collaborate with referral sources to extend Agrace's services. A valid WI RN license and at least one year of nursing experience are preferred.

Formación

• Current, unrestricted RN license in the state of employment.
• One year of nursing experience preferred.
• Ability to develop, maintain, and apply hospice knowledge in care planning.
• Strong communication and written skills.
• Valid driver's license and reliable vehicle.

Responsabilidades

• Assess patients and family needs and coordinate hospice care.
• Evaluate eligibility for hospice and assist with admissions.
• Develop and revise the plan of care with the IDT and family.
• Provide interventions, education, and pain management as needed.
• Oversee CNA/LPN and coordinate visits with the care team.
• Collaborate with referral sources to promote Agrace services.
