Job summary

Agrace seeks a Homecare Social Worker in Eau Claire, WI, to provide direct social work services within the patient/family-centered care model. The role involves assessments, plan-of-care updates, and ensuring smooth communication with the IDT.

Responsibilities include education, intervention, and resource referrals, while meeting regulatory standards and Agrace policies. On-call and holiday rotations are included.

Qualifications

• Bachelor’s in Social Work (BSW) required; Master’s in Social Work (MSW) preferred.
• Current, unrestricted Social Work License in state of employment.
• One-year paid medical social work experience.

Responsibilities

• Conduct assessments of patients and families; develop and update the Plan of Care (POC).
• Collaborate with the Interdisciplinary Team to coordinate homecare planning.
