Job summary

Agrace in Milwaukee is seeking an experienced registered nurse to join our hospice and palliative care team. You will assess patients and families, coordinate with an interdisciplinary team, and develop personalized plans of care to guide end-of-life care with compassion.

The role requires strong clinical judgment, excellent communication, and the ability to educate families about hospice services. Day shift, 40 hours per week, with occasional weekend rotations and travel as needed.

Qualifications

• Current, unrestricted Registered Nursing License in state of employment.
• One-year nursing experience, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Ability to develop, maintain, and apply knowledge of hospice and end of life process.
• Ability to maintain professional boundaries with patients, families, and co-workers.
• Ability to create and follow plans of care, and patient, family, and clinical staff directions.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills; Epic preferred.
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsibilities

• Patient and Family Assessments and Evaluations.
• Review medical records to identify hospice and palliative care services eligibility, physician orders, and to verify diagnosis.
• Collaborate with IDT and primary care provider to create HES for hospice eligibility.
• Identify patient needs and collect data from patient, family, and care team.
• Serve as a consultant to patients and families about end-of-life issues and Agrace programs.
• Document and report significant changes in condition to IDT.
• Oversee work of CNA/LPN and manage visits.
• Participate in meetings and precept new staff.
