Job summary

Agrace Health Inc is seeking a part-time Hospice Registered Nurse in Eau Claire, WI. You will assess patients and families, manage pain and symptoms, and collaborate with the interdisciplinary team to implement and revise plans of care. Weekend shifts (Sat/Sun) 8:00 am–4:30 pm, 20 hours per pay period, with two ten-hour shifts.

Requirements include a current RN license, 1 year nursing experience preferred, valid driver’s license, and strong communication and documentation skills in hospice care.

Qualifications

• Current, unrestricted RN license in the state of employment.
• One-year nursing experience preferred.
• Valid driver’s license and access to reliable vehicle.
• Ability to develop and apply knowledge of hospice and end-of-life processes.
• Ability to maintain professional boundaries with patients, families, and staff.
• Ability to create and follow plans of care with clinical staff directions.

Responsibilities

• Assess patients and family needs per the plan of care.
• Respond to changes in condition and provide acute patient interventions as needed.
• Provide interventions, treatments, and pain management.
• Evaluate effectiveness of pain management and adjust as needed.
• Educate patients and families on pain, symptom management and end-of-life care.
• Develop, implement, and revise individualized plans of care with the IDT and family.
• Oversee CNAs and LPNs and delegate visits as appropriate.
