Job summary

Four Pillars Hospice is seeking a Nurse Practitioner to oversee clinical care and coordinate palliative treatment in collaboration with the hospice physician. You will guide the IDG/T team, educate patients and families, and participate in care planning and on-call coverage.

The role requires NP licensure, prior APN experience (prefer hospice/home health), and strong knowledge of pharmacology and end-of-life care. A valid driver's license and flexible travel are expected.

Qualifications

  • Graduate from a baccalaureate nursing program and NP-focused graduate degree.
  • Current state RN license and Advanced Practice Nurse license.
  • Two years of APN experience; hospice/home health preferred.
  • Solid pharmacology, palliative care, and end-of-life care knowledge; strong interpersonal skills.
  • Valid driver's license and reliable transportation.
  • Ability to work a flexible schedule and travel locally.
  • Calm demeanor under stress; quick decision making for patient and family needs.
  • Adherence to HIPAA and hospice policies.

Responsibilities

  • Collaborate with the IDG/T to evaluate and provide palliative care.
  • Assist in developing patient/family plans of care for quality care.
  • Educate patients and families about disease processes and end-of-life care.
  • Provide staff training as needed.
  • Perform and document patient visits per plan of care.
  • Communicate with physicians about patient needs and outcomes.
  • Offer on-call services as assigned.

Job description

Position Summary

The Nurse Practitioner's Primary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients' attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care

Key Responsibilities
  • Collaborate with the IDG/T in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consult with and assist in the IDG/T's development of patient/family plan of care to assure quality care is provided.
  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
  • Perform and document visits to patient/family based on plan of care and patient/family need.
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
  • Consult with and provide education to other IDG/T members on a prn basis for clinical issues relating to palliation.
  • Activelyparticipate in the IDG/T. Communicate effectively, timely, and appropriately at all times.
  • Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
  • Complete face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
  • Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
  • Always uses standard precautions, and per hospice policy.
  • Adhears to hospice policies regarding protected health information.
  • Keep up with and maintains education/knowledge in pharmacology and palliative medicine.
  • Provide on-call services to patients/families as assigned.
Qualifications
  • Graduate from a Baccalaureate School of Nursing, a Nurse Practitioner (NP) focused Master's or Doctoral Nursing Program
  • Currentstate license as a Registered Nurse, and as an Advanced Practice Nurse. Current driver's license.
  • Twoyears' experience as an Advance Practice Nurse, clinical care setting preferred; hospice or home health preferred.
  • Nursingskills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills..
  • Transportation: Reliable transportation and valid auto liability insurance.
