Job summary

568 Hospice of Cabarrus County, Inc. is seeking a full-time RN to manage an assigned patient caseload in home health, with emphasis on timely care, documentation, and MD orders. The role requires accurate data collection and coordination of a multidisciplinary team to ensure high-quality outcomes.

The ideal candidate holds an RN license with BLS, BSN preferred, and EMR experience. This position offers a sign-on bonus up to $3,000 and a comprehensive benefits package, with a Mon-Fri 8:30am-5:00pm

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• BLS required per policy guidelines.
• Current RN license or temporary license as a Registered Nurse Practitioner in the state where you work.

Responsibilities

• Manage an assigned patient caseload in the home setting with timely care and documentation.
• Collect data from assessments and EMR for analysis and planning.
• Identify high-risk patients and coordinate with multidisciplinary team.
• Direct care coordination and supervision of LPNs and HHAs.
• Perform treatments and evaluations per policy and regulatory standards.
