Job summary

Advocate Health's Cabarrus region seeks an experienced RN for full-time home health. You will manage a patient caseload, conduct assessments, collect data from home visits and EMR, and coordinate a multidisciplinary team to meet care plans.

Eligible for a $3,000 sign-on bonus; internal referral bonus of $500. Base pay ranges from $35.50 to $53.25 per hour, with comprehensive benefits and 40-hour weeks in a M-F schedule.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• BLS required by policy.

Responsibilities

• Manage a patient caseload with timely care and documentation per plan of care and MD orders.
• Collect data from home visits, telephonic contacts, and EMR.
• Identify high-risk patients and coordinate care to mitigate risk and optimize outcomes.
• Prioritize multi-issue situations to ensure safety and timely services.
• Develop and communicate an effective plan of care to involved parties.
