Job summary

Atrium Health in Charlotte, NC is seeking a Home Health Registered Nurse to oversee an assigned patient caseload in the home setting, coordinating with a multidisciplinary team to ensure safe and effective care. This role requires an active RN license with BSN preferred and entails Mon–Fri hours with on-call and weekend rotation.

Sign-on bonus eligible up to $15,000 for qualified applicants; internal rotation or referrals may apply.

Qualifications

• RN license is required.
• BSN preferred but not mandatory.
• Experience in home health nursing or equivalent is preferred.

Responsibilities

• Manage daily patient caseload in the home setting.
• Collect data from patient assessments and EMR to inform care.
• Analyze data to identify risks and determine appropriate interventions.
• Coordinate care with a multidisciplinary clinical team.
• Ensure the plan of care is communicated, implemented, and documented.
