Job summary

Optum is seeking a Registered Nurse Case Manager for in-home hospice visits in North Carolina. You will evaluate patients, develop plans of care, and coordinate with the care team to promote holistic well-being. Travel within Harnett and Lee counties is required; weekend on-call may be needed.

Required RN licensure in NC and a valid driver's license with access to reliable transportation. CPR certification (or within 90 days) is preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Driver's license, valid vehicle insurance, and access to a dependable vehicle or public transport.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care; documents care and patient responses.
• Collaborates with patient/family, physicians and IDG; educates in self-care when appropriate.
• Maintains accurate clinical notes; ensure care regardless of patient demographics.

Education

RN licensure in North Carolina
