Qualifications

• Diploma, ADN or BSN from an accredited institution.
• Current Maryland RN license.
• Two years of nursing experience, or equivalent education and experience.

Responsibilities

• Identify patient/family care needs and provide nursing and supportive care.
• Complete initial patient/family assessment for home care needs.
• Prepare and implement the care plan with the Interdisciplinary Group.
• Provide direct nursing care in coordination with the primary physician.
• Plan and supervise care delivery by the HHA; use problem-oriented charting.
