Qualifications

• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledgeable of universal precautions and safe nursing practices.
• Ability to educate patients and families.

Responsibilities

• Completes initial assessment of patient and family to identify home care needs.
• Prepares and implements the care plan in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to maintain the highest level of care.
• Plans and supervises delivery of care by the Home Health Aide (HHA).
• Cooperates with Clinical Manager and outside agencies; updates physician and care team monthly.
• Uses problem-oriented charting to ensure continuity of care.
• Teaches family, facility staff, and caregivers nursing skills and medication administration.
• Ensures equipment needs in the facility setting are met.
• Attends to patient, family and facility at time of death.
• Maintains professional relationships with facility staff; participates in care plan meetings.
