Job summary

GBMC Hospice Home Care is seeking an experienced Registered Nurse to provide specialized home and facility-based care for terminally ill patients and support to families. This role emphasizes compassionate, patient-centered planning and collaboration with an interdisciplinary team.

The RN will assess patient and family needs, develop care plans, administer nursing care, supervise HHAs, and coordinate with providers. Strong communication, time management, and EMR skills are essential.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs (physical, emotional, spiritual, mental).
• Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients to maintain the highest level of care with the primary physician.
• Plans and supervises the delivery of care by the Home Health Aide (HHA).
• Coordinates with Clinical Manager and other teams, updates physician and interdisciplinary team as needed.
• Documents using problem-oriented charting to ensure continuity of care in home or facility settings.
• Teaches family, facility staff and caregivers nursing skills including analgesia, injections, IV care, dressing changes, and vital signs.
