GBMC HealthCare in Maryland seeks a Hospice Home Care Nurse to deliver compassionate nursing and supportive care to terminally ill patients and their families in home or facility settings. You will perform initial patient assessments, develop and implement care plans with the interdisciplinary team, and provide direct nursing care under physician orders.

The role requires a valid Maryland RN license, ADN/BSN, and at least two years of nursing experience, with strong communication and EMR skills

Qualifikationen

• Diploma, ADN and/or BSN from an accredited institution.
• Maryland RN license is required.
• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledgeable of universal precautions for infection control and safe nursing standards.

Aufgaben

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan with the Interdisciplinary Group.
• Provides direct nursing care to patients under physician orders.
• Plans and supervises the delivery of care by the HHA.
• Cooperates with Clinical Manager and updates physician and team as needed.
• Uses problem-oriented charting to ensure continuity of care in various settings.
• Teaches family and facility staff patient care skills (RN titrates meds, injections, IVs, etc).
• Ensures equipment needs for the facility are arranged.
• Attends to patient, family and facility at time of patient’s death.
