Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to assess patient and family needs, develop and implement care plans, and provide direct nursing support in home or facility settings. You will collaborate with the Interdisciplinary Group to coordinate patient-centered care and ensure safe, effective symptom management.

Responsibilities include teaching families, coordinating with facility staff, and maintaining thorough chart documentation.

Qualifications

• Diploma in Nursing or ADN/BSN from an accredited program.
• Active RN license in Maryland.
• Minimum of two years of nursing experience.
• Experience with EMR systems preferred.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan for patient/family in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients to maintain the highest level of care.
• Plans and supervises delivery of care by home health aides.
• Updates physician and interdisciplinary team on patient status monthly.
• Uses charting to ensure continuity of care in home or facility settings.
• Educates family and staff on care techniques and safety.
• Arranges necessary equipment for facility care and attends to patient at time of death.
