Job summary

Continuum Hospice & Palliative Care California is seeking a self-driven Community Liaison to join our San Jose area team. This role focuses on building trust-based relationships with referral sources, physicians, and healthcare partners to grow census and promote our hospice services.

You'll conduct regular outreach, advocate for compassionate care, and drive growth while serving as the face of Continuum Hospice.

Qualifications

• Bachelor’s degree in related field preferred.
• Minimum two years of sales/marketing experience in healthcare.
• Hospice/palliative marketing experience preferred.
• Must have reliable transportation and valid driver’s license.

Responsibilities

• Establish and nurture relationships with referral sources, physicians, and healthcare partners to grow census.
• Conduct regular outreach to advocate for hospice services and compassionate care.
• Position Continuum Hospice as provider of choice across the service area to exceed growth goals.
• Drive referral growth through visits with referring physicians, discharge planners, nursing facilities, social workers.
• Lead initial conversations about hospice care and facilitate informed consent with empathy.
